BOTOX is a toxin that temporarily weakens the small muscles of facial expression. After a successful treatment the muscle hardly contracts so the skin lying above it lies flat. Treating crow’s feet is an example of this use of Botox. Depending on the location and depth of the crease, permanent filler will dramatically improve the long term outcome. The deep vertical creases that develop just inside the eyebrows first need Botox, then one week later, silicone. If I just used silicone, the muscle will squeeze the filler out of place before the body isolates and anchors the droplet. The biggest flaw with Botox is that your body metabolizes and absorbs it after 3-6 months. The good news is that silicone, injected properly, is permanent.
FILLERS. There are two classes of fillers, Absorbable and Permanent. The absorbable kind is safe and rather simple to use. The flaw....you will need to re-inject it every 3-6 months. A permanent filler is silicone. The FDA reports that it is safe and an independent science panel found that it has no links to any systemic disease. I have done thousands of treatments over the decades and have not had a single complaint of infection, rejection, hardening or migration. I started my career as a Head and Neck surgeon and now, as a clinical assistant professor, Department of Surgery, I share my insights with my students. The most important factors in determining the success of this filler are the talent and experience of the doctor. The silicone must be fanned out in micro-droplets and placed at the exact depth. Volume must be added at a conservative rate - it is easier to add a few drops than remove!
